Overview
The peer review process is a structured, HIPAA-compliant approach to reviewing cases identified through the mortality committee, risk management, patient safety initiatives, or brought forward by colleagues. The primary goal is to promote learning and system improvement rather than assign blame. Cases are presented in a respectful, non-punitive manner that supports a culture of safety and professional growth. The focus is on understanding what went wrong and how care can be improved, rather than identifying who is at fault. This process encourages open discussion, reflection, and shared learning among clinicians to enhance patient outcomes and prevent future errors.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Recognize opportunities for practice improvement, including individual, team, and system level interventions, based on peer review findings.
  2. Discuss documentation and follow up strategies to close the loop on peer review cases and ensure accountability and learning integration.
  3. Provide and receive constructive feedback using respectful, nonpunitive language to support a culture of learning and continuous improvement.
  4. Distinguish between systems-based and individual-level factors that contribute to diagnostic or management challenges in primary care.
